Job summary

Madrona Hospice Care Inc. is seeking a Medical Social Worker to shape home-based patient care and mentor a team of social workers.

This per diem role focuses on clinical supervision, coordination with other providers, and ensuring high-quality service across our organization. You will engage in policy development, care planning, and community resource linkage, with an emphasis on compassionate support for patients and families in crisis.

Qualifications

• MSW from an accredited program.
• Active California LCSW license.
• ≥3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication and trust-building with patients and staff.
• Self-directed, organized, and collaborative mindset.

Responsibilities

• Lead daily operations of the social services team including policies and care coordination.
• Recruit, onboard, and mentor social workers with clinical supervision.
• Oversee assessment, planning, and evaluation of social services care.
• Partner with the Executive Director on annual department budget and resources.
• Build knowledge of community resources and connect patients to support.
• Plan in-service training and continuing education for the team.
• Contribute to quality improvement and explore new patient services.
• Perform other duties as required for patient services.
