Job summary

Madrona Hospice Care Inc. is seeking a Medical Social Worker to lead our home-based social services, mentor a team of social workers, and set the standard for compassionate care across the organization.

You will supervise daily operations, ensure compliant documentation, collaborate with clinical staff, and help plan budgets and resources while delivering patient- and family-centered support in a per-diem role with a rate of $100–$120 per visit.

Qualifications

• MSW from an accredited program.
• Active CA LCSW license required.
• At least 3 years of management experience.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services
