Per diem position, pay rate $100–$120 per visit. Requires MSW, California LCSW, and at least 3 years of management experience in home care, with strong knowledge of healthcare regulations and excellent communication.

Qualifications

• MSW degree required from an accredited program.
• Active California LCSW license required.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures, and care coordination with clinical providers.
• Recruit, onboard, and mentor social workers, providing supervision and supporting professional growth.
• Oversee assessment, planning, and evaluation of social services care, ensuring documentation meets state/federal/ACHC standards.
• Partner with the Executive Director on departmental budget and resource planning.
• Build knowledge of community resources and connect patients and referrals to the right support.
• Plan in-service training and continuing education to keep skills sharp.
• Contribute to quality improvement initiatives and identify new ways to better serve patients.
• Perform other duties as required for patient services.
