Job summary

Madrona Hospice Care Inc. is seeking a Medical Social Worker (per diem) in California to shape home-based patient care, mentor a team of social workers and uphold high-quality service standards across the organization.

The role requires an MSW, a California LCSW license, and at least 3 years of management experience in home care. You will facilitate care coordination, policy implementation, and adherence to regulatory standards while delivering compassionate support to patients and families.

Qualifications

• Proven ability to lead a social services team in a hospice setting.
• Strong communication with patients, families and staff while maintaining compassionate care.
• Experience mentoring social workers and overseeing clinical practice.

Responsibilities

• Lead daily operations of the social services team, policies, procedures and care coordination.
• Recruit, onboard, and mentor social workers with clinical supervision and growth support.
• Oversee assessment, planning, and evaluation of social services care ensuring compliance with state/federal/ACHC standards.
• Partner with the Executive Director on annual department budget and resource planning.
• Build knowledge of community resources and connect patients and referral sources to support.
• Plan in-service training and continuing education to keep team skills sharp.
• Contribute to quality improvement initiatives and identify ways to better serve patients.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $100 - $120 per visit.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
• A self-directed, organized, and flexible approach—you take initiative and work well with others
