Job summary

Madrona Hospice Care Inc. is seeking a Medical Social Worker to shape home-based patient care, mentor a team of social workers, and uphold high standards of compassionate service.

You will lead daily operations of the social services team, oversee assessment, planning, and evaluation, ensure state, federal, and ACHC standards, and partner with leadership to align resources with patient needs. This per diem role offers a flexible schedule with the potential to convert to full-time based on

Qualifications

• MSW from an accredited program.
• Active California LCSW license.
• At least 3 years of management experience, preferably in home care.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures, and care coordination with clinicians
• Recruit, onboard, and mentor social workers—providing clinical supervision and supporting professional growth
• Oversee assessment, planning, and evaluation of social services care; ensure documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build knowledge of community resources and connect patients and referrals to support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and identify ways to better serve patients
• Perform other duties as required for patient services
