Job summary

Outfield Healthcare Partners is seeking an experienced MDS Coordinator to join our fully staffed facility. This role will oversee timely completion of the RAI and care management processes from admission to discharge, under direction from the Director of Nursing and VP of Clinical Reimbursement.

You will collaborate with the Interdisciplinary Team to review resident needs, ensure regulatory compliance, and maintain accurate MDS coding and documentation. Experience with MDS 3.0 is preferred.

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Thorough understanding of OBRA regulations and MDS.
• Knowledge of the care planning process.
• Experience with MDS 3.0 preferred.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the Interdisciplinary Team.
• Ensure team decisions on MDS, assessments per clinical criteria and RAI User's Manual.
• Assist with coordination of daily stand-up meetings and assessment reference dates.
• Comply with federal and state regulations for RAI processing.
• Monitor MDS and care plan documentation in the medical record.
