Job summary

Valhalla Post Acute in Louisville is seeking an experienced LPN/LVN to oversee nursing assistants and ensure compliance with facility policies. You will supervise daily nursing activities, maintain accurate documentation, and participate in discharge planning to coordinate resident care and services.

Ideal candidates will have an active LPN/LVN license, nursing degree, strong leadership, and experience with PCC and EMR systems.

Qualifications

Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
Must be able to read, write, speak, and understand the English language.

Responsibilities

Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by this facility.
Participate in facility surveys (inspections) made by authorized government agencies as may be requested.
Develop and maintain a good working rapport with inter-departmental personnel to ensure nursing services meet residents’ needs.
