Job summary

This role requires an active LPN/LVN license in NC, CPR/BLS preferred, at least one year of clinical nursing experience, and familiarity with EHR systems.

Qualifications

• Active LPN/LVN license in North Carolina
• Minimum one year of clinical nursing experience; long-term care preferred
• CPR/BLS certification preferred

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety
• Supervise and direct nursing assistants and medication aides during shift
• Monitor residents' condition and report changes to the physician
• Develop, implement, and review individualized care plans with the interdisciplinary team
• Complete accurate clinical documentation per policy
• Coordinate admissions, transfers, and discharges
• Participate in quality assurance and regulatory surveys
