Avamere Skilled Advisors, LLC seeks a licensed practical nurse (LPN) for our memory care and assisted living facility at Avamere at Bethany in Portland, OR. You will administer medications, monitor residents, and supervise CNAs under an RN's direction.

The role emphasizes dementia-focused care, care-plan execution, regulatory compliance, and effective communication with families and doctors. Day shifts, Sunday-Thursday or Tuesday-Saturday, full-time.

Formación

Current LPN license in Oregon.
Experience in assisted living or memory care.
Knowledge of CMS and state regulations.
Ability to supervise CNAs and care staff.
Experience with electronic medical records.

Responsabilidades

Medication administration: Safely prepare, administer, and document medications and treatments.
Health monitoring & assessments: Monitor vital signs and health changes; conduct routine evaluations.
Memory care focus: Provide dementia-focused care and redirection.
Care plan execution: Follow ISPs and adjust as needs evolve.
Staff supervision: Guide CNAs and caregivers during daily routines.
Family & doctor liaison: Communicate updates to families and physicians.
Resident assessments: Assess needs and update care plans accordingly.
Training & mentorship: Educate staff on dementia care and procedures.
Daily oversight: Ensure adherence to structured routines.
