Job summary

Arete Living is seeking an LPN for memory care and assisted living placement at Avamere Bethany in Portland, OR. The LPN will deliver direct medical care, manage medications, and support residents’ cognitive and physical needs under RN supervision.

Responsibilities include supervising CNAs, communicating with families and physicians, and ensuring ISPs are followed to maintain a warm, safe environment for memory care residents on day shifts (Sunday-Thursday or Tuesday-Saturday).

Qualifications

• Must hold an active LPN license in the state.
• Experience in memory care or Alzheimer's/dementia care preferred.
• Ability to supervise CNAs and frontline staff effectively.
• Strong communication with families, physicians, and team members.
• Knowledge of CMS/state regulations for long-term care.

Responsibilities

• Administer, document, and monitor medications and treatments.
• Assess residents and track health changes; monitor vital signs.
• Provide memory-care focused care and redirection.
• Execute care plans (ISPs) with RN oversight and adjustments.
• Supervise CNAs and caregivers; mentor staff.
• Liaise with families and physicians about residents’ status.
