Job summary

Avamere Living is seeking a Licensed Practical Nurse (LPN) to join our memory care and assisted living team in Portland, OR. This full-time day-shift position involves direct medical care, medication management, and supervising frontline caregivers under RN supervision.

The ideal candidate brings LPN licensure in Oregon, experience in senior care, and strong leadership and communication skills to support residents and families while adhering to CMS and state regulations.

Qualifications

• Must possess a nursing degree from an accredited college.
• Current, active LPN license to practice in Oregon.
• Experience in assisted living, memory care or geriatric settings.
• Knowledge of CMS and State regulations regarding patient care.
• Experience with electronic medical records documentation.
• Strong leadership and communication skills.
• Maintain CPR/BLS during employment.
• Confidentiality and compassionate elder care commitment.

Responsibilities

• Medication administration, documentation, and narcotic counts.
• Monitor residents' health, vital signs, and routine assessments.
• Memory care focus with dementia-specific care and behavioral support.
• Follow ISPs and care plans designed by wellness director or RN.
• Supervise CNAs and care staff, providing guidance daily.
• Communicate with physicians and families regarding updates.
• Conduct resident assessments and update care plans as health changes.
• Provide staff training and mentorship on best practices.
• Ensure daily routines support memory care residents.
• Advocate for residents and connect with community resources.
