Job summary

Sonoma Post Acute in Citrus Heights, CA is seeking an LVN/LPN for a full-time role to provide direct nursing care to residents and supervise nursing assistants. You will ensure care plans are followed, administer medications, document accurately, and maintain excellent clinical standards under the supervision of the Director of Nursing.

The position requires an active LPN/LVN license in California, current CPR certification, and the ability to coordinate with medical staff.

Qualifications

• Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
• Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
• Current CPR certification.
• Knowledge of PCC preferred.

Responsibilities

• Direct the day-to-day functions of the nursing assistants.
• Administer medications as ordered by the physician and ensure accurate documentation.
• Supervise nursing staff and ensure compliance with facility policies and procedures.
• Assist in planning shifts, services, programs, and activities for residents.
• Maintain patient safety and contribute to the facility's quality assurance program.
