Qualifications

• Bachelor's Degree in Nursing or Social Work required/preferred.
• RN license preferred; LVN/LPN license acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs/benefits.
• PCC knowledge preferred.
• CCM certification a plus.
• Continuing education/licensing must be maintained.

Responsibilities

• Communicate plans of care with residents and families.
• Attend morning stand-up meetings to coordinate care.
• Explain care plans, treatments, and illness progression to residents.
• Monitor resident status and adjust as needs change.
• Prioritize daily tasks for admissions, discharges, and ongoing needs.
• Complete documentation and maintain charting.
• Perform utilization review to ensure cost-effective care.
• Coordinate referrals with residents, providers, and payers.
