Job summary

Fountain Valley Post Acute seeks an experienced LPN/LVN to provide direct nursing care and supervise nursing assistants, ensuring admission, discharge, and daily care plans are executed per policy. You will maintain documentation, coordinate with physicians, and lead shift staffing.

Must hold an active CA nursing license and CPR certification. The role requires strong leadership, exceptional communication, and the ability to maintain a calm, compassionate environment for residents and families

Qualifications

• Must be able to read, write, speak, and understand English.
• Must possess Nursing Degree from an accredited college and an active LPN/LVN license in the state.
• Current CPR certification.
• Experience in nursing service facilities preferred.

Responsibilities

• Direct the day-to-day functions of nursing assistants.
• Ensure compliance with policies and procedures.
• Lead staff meetings to plan shifts and services.
• Maintain Nursing Service Procedures Manual.
• Complete required forms and reports for resident care.
• Admit, transfer and discharge residents as required.
• Administer medications as ordered by the physician.
• Supervise nursing personnel and CNAs.
• Coordinate with other departments for resident care.
