The role includes supervising nursing staff, coordinating care plans, and ensuring accurate documentation while adhering to state and federal regulations.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• Minimum one year of clinical nursing experience; long-term care preferred.
• Supervisory or charge nurse experience preferred.
• CPR/BLS certification preferred.
• Experience with electronic health records (EHR) and medical documentation.
• Strong communication and teamwork skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' condition and notify physician of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Maintain accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
