Job summary

St. Croix Hospice is seeking an RN Clinical Supervisor to coordinate, manage and deliver hospice care. You will supervise interdisciplinary teams, review cases, set goals and plans of care, participate in case conferences and ensure compliance with laws.

This role requires an active RN license, ability to work in a home-based, interdisciplinary environment and on-call availability. Ideal candidates have an accredited nursing degree (BSN preferred) and experience in hospice/home care with

Qualifications

• Must have current RN license in state(s) of practice.
• Multistate license accepted where applicable.
• Graduate of an accredited school of nursing; BSN preferred.
• Ability to pass background and drug screening.

Responsibilities

• Receives case referrals and assigns hospice personnel.
• Reviews patient information and determines hospice needs.
• Conferences with Medical Director regarding eligibility for services.
• Reviews cases and guides clinicians to improve care.
• Participates in case conferences and coordination of interdisciplinary care.
• Maintains clinical records compliant with laws and policies.
• Assists in orientation and education of staff.
• On-call duties as defined by policy.
