St. Croix Hospice in Missouri is seeking an experienced RN Clinical Supervisor to coordinate patient care and lead care teams across hospice services.

You will review referrals, assign clinicians, and collaborate with Medical Director to ensure plans of care meet patient needs; you'll guide and mentor staff while maintaining compliant records. This role reinforces our commitment to compassionate, high-quality end‑of‑life care in the community.

Qualifications

• Must have a RN license in state of practice.
• Graduate of an accredited nursing program; associate degree required.
• Proven ability to work within an interdisciplinary setting and pass background checks.

Responsibilities

• Review referrals and assign hospice personnel to cases.
• Confer with Medical Director or Hospice Physician on eligibility and care plans.
• Be available during operating hours to assist clinicians as needed.
• Review patient diagnosis, prognosis, medications and clinical course.
• Assist in establishing immediate and long-term goals and plans of care.
• Attend case conferences to coordinate care and interdisciplinary involvement.
• Conduct quarterly record reviews and report findings to the Manager of Clinical Services.
• Assist in new employee orientation and onboarding.
• Ensure clinical records comply with applicable laws and policies.
