The role requires an active RN license, bachelor’s degree preferred, and experience in hospice or homecare settings. Strong leadership, organizational and communication skills are essential.

Qualifications

• Current RN license in state(s) of practice.
• Graduate of an accredited school of nursing.

Responsibilities

• Coordinate and assign hospice personnel to cases.
• Review patient information to determine home care needs.
• Attend case conferences with Medical Director and clinicians.
• Conduct quarterly record reviews and communicate findings.
