Job summary

Affinity Health Management in Warren, OH seeks a Nurse Practitioner to provide comprehensive hospice services under physician supervision. Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• A current DEA license is required.
• Current nursing licensure in the state and CPR certification.
• Master’s degree with a minimum of one year NP experience and hospice or palliative care experience.
• Accreditation from an approved certifying body for advanced practice nursing as required by state.
• Specialist area certification (e.g. CHPN) preferred.

Responsibilities

• Provides medical care and manages terminal illness under physician supervision.
• Performs history and physical exams and documents findings.
• Develops an individualized plan of care and prescribes treatments.
• Completes hospice face-to-face visits and assesses eligibility with the team.
• Assists with hospice eligibility certification and recertification decisions.
• Oversees patient care in consultation with the hospice interdisciplinary team.
• Ensures symptom control through collaboration with team members.
• Discusses treatment options and adjusts plans with collaborating physician.
• Consults with physician when plans fall outside standard practice.
