Job summary

The Hospice Home Care Nurse at Greater Baltimore Medical Center (GBMC) provides compassionate nursing and supportive care to terminally ill patients and their families in home, facility, and respite settings across Baltimore County.

Responsibilities include initial assessments, care planning with the Interdisciplinary Group, direct nursing care, staff supervision of HHA, and education for families. Maryland RN license and ADN/BSN diplomas are required; two years of nursing experience preferred.

Qualifications

• Two years of nursing experience, or equivalent combination of education and experience.
• Knowledgeable of universal precautions for infection control and safe nursing practices.
• Strong time management skills and patient-focused care.
• Excellent interpersonal and communication skills are required.
• Ability to be patient- and family-centered;EMR skills or ability to learn.

Responsibilities

• Completes initial assessment of patient and family to identify home care needs.
• Prepares and implements the care plan with the Interdisciplinary Group.
• Provides direct nursing care to maintain the highest level of patient care.
• Plans and supervises delivery of care by the HHA.
• Updates physicians and interdisciplinary team; documents in charts.
• Educates family and facility staff on care tasks, meds, and vitals.
