Qualifications

Responsibilities

• Conducts initial assessment of patient and family to determine care needs.
• Implements care plans in collaboration with the interdisciplinary group.
• Provides direct nursing care in conjunction with the primary physician.
• Plans and supervises care delivery by other team members.
• Trains family members and facility staff on care skills.
• Attends to patients at the time of their death.
