Job summary

GBMC HealthCare is seeking a Hospice Home Care Nurse to provide compassionate nursing and supportive care to terminally ill patients and their families in home settings. You will perform assessments, implement care plans, and coordinate with an interdisciplinary team.

Qualifications

• Registered Nurse with a Diploma, ADN or BSN from an accredited program.
• Maryland RN license is required.
• Two years of nursing experience or equivalent education/experience.
• Strong communication, time management and patient-family centered care.

Responsibilities

• Completes initial assessment of patient and family to identify home care needs.
• Prepares and implements the care plan with the Interdisciplinary Group.
• Provides direct nursing care in home settings and with the primary physician.
• Plans and supervises care delivery by home health aides.
• Updates physicians and interdisciplinary team; documents care.
• Tritrate medications within prescribed ranges and educate family.
• Arrange equipment and support for facility/home settings.
• Attend patient death with sensitivity and professionalism.
