Job summary

GBMC HealthCare seeks a Hospice Home Care Nurse to provide compassionate nursing and supportive care to terminally ill patients and their families in home and facility settings.

The role requires ADN/BSN, Maryland RN license, and at least two years of nursing experience. You will assess needs, develop care plans with the Interdisciplinary Group, educate families, and coordinate with the care team to ensure comfort and quality of life.

Qualifications

• Two years of nursing experience, or equivalent combination of education and experience.
• Current Maryland State license as a Registered Nurse.

Responsibilities

• Completes initial assessment of patient and family to ascertain home care needs.
• Prepares and implements the care plan for patient/family unit in collaboration with the Interdisciplinary Group.
• Provides direct nursing care to patients in order to maintain highest level of care as ordered by physician.
• Plans and supervises the delivery of care by the Home Health Aide (HHA).
• Cooperates with Clinical Manager in utilization of other team members and community resources; updates physician and interdisciplinary team regularly.
