Enhabit Home Health & Hospice is seeking an experienced registered nurse to serve as a supervisory leader in Augusta. The role coordinates patient care, ensures regulatory compliance, and drives collaboration among physicians, caregivers, and operations teams to deliver high-quality home health services.

The ideal candidate has at least 2 years of home health nursing, an active RN license, CPR certification, and a commitment to patient-centered care.

Qualifications

• Must be a graduate of an approved school of professional nursing.
• Must maintain a valid nursing license as required by state rules.
• Minimum two years nursing experience within the last 36 months.
• Minimum one year of home health experience.
• Knowledge of federal, state and local home care regulations.
• CPR certification required.
• Valid state driver license required.
• Automobile liability insurance required.
• Basic technology skills including use of a mobile device.
• Preferred: management experience and HCHB familiarity.
• Bilingual (English/Spanish) preferred.
• In-office attendance is essential unless designated full-time remote position.

Responsabilités

• Coordinate overall home health services for patients as per standards and procedures.
• Review and approve start of care, recertification, resumption of care, and evaluations.
• Facilitate collaboration between physicians, referrals, patients, caregivers and staff.
• Schedule patients with field clinicians and communicate with stakeholders to maintain care continuity.
