Job summary

Enhabit Home Health & Hospice in Columbus, GA seeks an experienced registered nurse to coordinate and supervise home health services. You will review patient data at key care milestones and facilitate collaboration among physicians, patients, and caregivers to ensure high-quality care.

The role requires an active RN license, CPR certification, and prior home health experience. Excellent communication and in-office collaboration are essential for success in this leadership position.

Qualifications

• Graduate of an approved school of professional nursing.
• Current, valid nursing license per state requirements.
• CPR certification required.
• Experience in home health preferred.
• In-office attendance is essential unless remote duties are approved.

Responsibilities

• Coordinate the overall coordination of home health services per standards and procedures.
• Review and approve patient information at start of care, recertification, resumption of care, or evaluation visit.
• Facilitate communication among physicians, referral sources, patients, caregivers, and employees.
• Ensure scheduling of patients to field clinicians and communication to maintain care continuity.
