Elara Caring in Worcester, MA, seeks a Behavioral Health Clinical Team Manager to lead a skilled team delivering home-based behavioral health care. You will mentor providers, coordinate care plans, and ensure timely starts of care while upholding regulatory standards.

You'll oversee assignments, integrate physician orders, and foster a compassionate, compliant team focused on quality outcomes and patient-centered solutions within our expanding home health program.

Qualificações

• Associate’s Degree in Nursing required; Bachelor’s preferred.
• Current state RN license.
• At least 2 years in Home Health or Behavioral Health; 1+ year in leadership.

Responsabilidades

• Lead Exceptional Care Delivery: Oversee and coordinate home health services tailored to behavioral health needs.
• Manage patient and provider assignments, ensure compliance with regulations, and prioritize timely starts of care.
• Champion Quality & Compliance: Approve and evaluate individualized care plans to maintain the highest clinical standards.
• Seamlessly integrate and coordinate physician orders.
• Build a Culture of Compassion: Inspire and support a team of healthcare providers, ensuring patient needs are continuously assessed and met with excellence.
• Streamline Operations: Manage processes for physician orders, staff communication, and interdisciplinary coordination for holistic care.
