Addus HomeCare in Medford, OR is seeking a Service Coordinator to schedule caregivers, supervise in-home staff, and manage client updates. The role involves ensuring care plans are followed, coordinating coverage, and maintaining service quality in a fast-paced environment.

The ideal candidate will have 3–5 years of industry experience, strong communication and organizational skills, and a valid driver's license to perform in-home supervisory visits.

Qualifications

• 3 to 5 years of industry experience required.
• Must have high school diploma or equivalent.
• Interpersonal, organizational and communication skills.
• Computer skills including Microsoft Word and Excel.
• Must have DL to complete in-home supervisory visits.

Responsibilities

• Creates work schedules by entering schedules into the system and manages changes.
• Provides alternate coverage to ensure the client’s care plan is followed.
• Contacts care providers and clients to provide service updates.
• Conducts monthly client wellness calls and home visits as required.
• Supervises direct service employees by setting attendance, performance and conduct expectations.
• Maintains a professional, neat and orderly branch environment.
• Maintains confidentiality due to sensitive information.
• Follows all regulations (MCO, Medicare, Medicaid, HIPAA) and company policies.
• Handles telephones, assists staff and greets visitors.
• Performs filing, scanning, data entry and reports.
