Job summary

WelbeHealth in California is seeking a Home Care Coordinator Supervisor reporting to the Home Care Manager to oversee a team of Home Care Coordinators, coordinating with the Home Services team and community partners to support frail seniors.

You will mentor staff, conduct case reviews, lead daily huddles, participate in enrollment efforts, and ensure documentation quality and compliant care plans in a compassionate, value-based program.

Qualifications

• Bachelor’s degree preferred.
• Healthcare licensure or equivalent experience required.
• CPR and First Aid certification within 180 days of hire.

Responsibilities

• Review and audit Participant Care Plans for compliance and consistency.
• Oversee pre-enrollment assessments and collaborate with MOE team.
• Manage direct reports including hiring, training, supervising and mentoring.
• Lead internal investigations and coordinate with HR for resolutions.
• Lead daily meetings with HCCs to discuss assessments and hospitalizations.
• Conduct coaching sessions and IDT case reviews with HCCs.
