Job summary

WelbeHealth in the United States seeks a Home Care Coordinator Supervisor reporting to the Home Care Manager to lead a team of Home Care Coordinators and collaborate with the Home Services group and community partners.

You will review participant care plans, oversee pre-enrollment assessments, hire and mentor staff, and steer IDT meetings to ensure compliant, high‑quality care while addressing complex situations with sensitivity and coordinated teamwork.

Qualifications

• Healthcare or medical licensure or equivalency, with an additional three (3) years of professional experience.
• Minimum of three (3) years of case management or nursing experience in a home setting with frail or elderly population.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Responsibilities

• Review and audit Participant Care Plans completed by HCCs to provide coaching and mentorship on documentation guidelines for compliance and consistency in Wellsky Personal Care and Athena.
• Oversee pre-enrollment assessments, collaborate with MOE team, and attend weekly meetings.
• Manage direct reports including hiring, training, supervising and mentoring.
• Lead internal investigations between Home Care Assistants and participants regarding conflicts and coordinate with HR for resolution.
• Lead daily meetings with HCCs to discuss pending assessments and hospitalizations.
• Conduct check-in sessions with HCCs to complete case reviews and propose solutions in IDT meetings.
