Hope Network is seeking a Lead Direct Care Professional to oversee, support, and directly care for individuals served. This role leads shift activities, models person-centered support, and ensures implementation of medical, clinical, and behavioral plans.

You will mentor new staff, assist with on-call responsibilities, and guide team members in supervisors' absence. The position emphasizes compassionate care, reliability, and strong communication within a collaborative team.

Responsibilities

• Ensure proper implementation of clinical and behavioral goals for each consumer.
• Maintain timely, accurate daily observational data and document in electronic records.

The Lead Direct Care Professional provides oversight, support, and direct care to individuals served by Hope Network across a variety of settings. This role serves as a leader on shift, modeling person-centered support, promoting independence, and ensuring the implementation of medical, clinical, therapeutic, and behavioral support plans. The Lead Direct Care Professional also mentors, trains, and orients new staff, assists with on-call responsibilities, and provides guidance and support to team members when supervisors are unavailable or off-site.

What You'll Do

• Regular and predictable attendance is an essential requirement of this position. Successfully complete all required and applicable trainings and in-services including annual refresher and appropriate intervention techniques.
• Provide leadership, direction and support to staff on shift in coordination with the supervisor. This may include, but not be limited to scheduling, training, compliance, on-call pager, and administrative supports.
• Elevate employee relations concerns to supervisor. May assist supervisor on special projects.
• Provide input as requested in the development of the consumer’s treatment plans and goals. Confirm treatment plans are implemented properly.
• Implement and participate in the clinical and behavioral goals for each consumer using approved methods of instruction and intervention.
• Provide through, accurate and on-time daily observational data and document using electronic record according to Agency standards.

What You Bring

Required:

• Bachelors degree with 2 years experience in applicable setting
• Associates degree with 3 years experience in applicable setting
• HS Diploma/GED with 4 years experience in applicable setting.
• Demonstrated critical thinking/problem solving skills to respond appropriately to medical and behavioral issues.
• Demonstrated leadership skills and ability to problem solve independently.
