Job summary

The Opportunity Tree in Phoenix, AZ is seeking a Lead Direct Support Professional to support Members with developmental disabilities in daily living skills and independence. You will supervise health and safety, coordinate daily living activities, arrange transportation, manage records, and ensure medication procedures align with the Person-Centered Plan.

Ideal candidates are 18+, fluent in English, with a HS diploma or GED, valid driver license, and willingness to complete required trainings

Qualifications

• Minimum 18 years of age.
• Valid driver license with acceptable MVR.
• Fluent in English, both spoken and written.
• Experience in IDD field preferred.
• High school diploma or GED required.
• Basic computer skills (Outlook, Word, Excel).
• Willingness to complete required trainings (CPR, First Aid).
• AZ DPS Fingerprint Clearance Card requirement.
• Reliable transportation and possible travel.
• Team player and good communication with Members, staff and families.

Responsibilities

• Supervise Members’ health, safety and security, including Activities of Daily Living.
• Coordinate instruction and supervision of daily living skills and recreational activities.
• Encourage Members to develop positive interpersonal skills.
• Coordinate transportation per Person Center Plan.
• Ensure proper medication procedures are followed and orders implemented.
• Assist to develop and monitor each Member’s program.
• Communicate progress and issues to staff in a timely manner.
• Maintain Member and household accounts and submit paperwork on schedule.
• Maintain all required records and documentation.
