Merakey is seeking a Lead Direct Support Professional in Stockton, CA to support individuals with disabilities in residential/community settings. The role emphasizes teaching, coaching, modeling, and ensuring participation in daily activities while upholding dignity and person-centered care.

Responsibilities include leading shifts, assisting with daily living activities, scheduling support, documenting progress, and following care and safety plans.

Qualifications

• Must be at least 18 years old.
• Must have desire to help others reach their fullest potential.
• Ability to read, speak, write and comprehend the English language.

Responsibilities

• Act in lead role on assigned shift; serve as point person for activities and incident reporting.
• Assist Administrator in staff scheduling as assigned.
• Support individuals in tasks of daily living and personal care.
• Assist with meals and nutrition
• May transport individuals and complete pre/post paperwork for medical appointments.
• Document daily progress in the health record and communication logs.
