Job summary

Hope Network is seeking a Lead Direct Care Professional to provide oversight, support, and direct care across settings. You will model person-centered care, promote independence, and ensure behavioral and clinical support plans are followed.

You will mentor staff, participate in on-call duties, and guide teams when supervisors are unavailable. This role emphasizes leadership and compassionate teamwork to help individuals build independence.

Responsibilities

• Lead on-shift supervision and support with scheduling, training, compliance and on-call duties.
• Mentor and orient new staff; assist with on-call responsibilities.
• Provide input to treatment plans and ensure plans are implemented correctly.
• Implement and participate in clinical and behavioral goals for each consumer.
• Provide accurate daily observational data and document according to agency standards.
• Elevate employee relations concerns to supervisor and assist with special projects.
