Job summary

Washburn County, Wisconsin is seeking a CCS Service Facilitator to support children eligible for CLTS and CCS programs. Under supervision, you will help families identify, secure, and monitor services to meet outcomes and maintain home or community participation.

Bachelor’s degree in Social Work or related Human Services field required; experience in mental health/CLTS case management preferred.
Must have access to private transportation for work-related duties and a valid Wisconsin driver’s license.

Responsibilities

Assists children and families in identifying, securing, and monitoring services to meet outcomes.
Conducts comprehensive, strength-based assessments through interviews, home visits, and collateral information gathering.
Develops and updates service plans in collaboration with consumers and supports.
Provides crisis and short-term intervention and maintains documentation.
Works with a multidisciplinary team to deliver recovery-focused services.
