Job summary

Qualifications

• Bachelor's degree in counseling, social work, psychology, or human services required.
• Social Work Certification preferred.
• Experience with children, youth, and families in related systems is essential.

Responsibilities

• 40%: Conduct functional screens and assessments for children and coordination with other teams.
• 20%: Complete required case management documentation including eligibility and contacts.
• 15%: Follow up with children in crisis and coordinate care with partners.
• 15%: Answer phones and emails daily and provide information to families.
• 5%: All other duties as assigned.
