Qualifications

• Graduate of an approved school of professional nursing with an active license.
• Maintain a valid clinical license per state requirements.
• Minimum two years of nursing experience in the last 36 months.
• Minimum one year of home health experience.
• Knowledge of federal/state/local laws governing home care operations.
• CPR certification and a valid state driver license.
• Auto liability insurance as required by law.
• Basic technology skills including mobile device usage.
• Preferred: management experience and Homecare Homebase (HCHB) familiarity.
• Preferred: bilingual English/Spanish for clear patient communication.
• Reliable transportation; ability to drive in various weather conditions.
• Strong oral and written communication; proficient in English; active listening.
• Proficient with Microsoft Office (Word, Excel).

Responsibilities

• Coordinate overall home health services for the patient according to standards and procedures.
• Review and approve patient information during start of care, recertification, resumption, or evaluation visits.
• Facilitate relationships among physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and ensure clear communication with all stakeholders to maintain continuity of care.
