Pay Range: Up to $80,000 + $2,000 Sign-on Bonus
The Position:
Seeking a full-time Lead Clinical Care Manager with experience in home-care, office healthcare, care coordination, team development, or clinical education. This role involves leading the Clinical Care Manager team to clinical success, including:
- Collaborating with Clinical Care Managers and Leadership
- Serving as a clinical subject matter expert and preceptor
- Managing resources to meet regulatory deadlines
- Monitoring quality metrics and daily operations
- Assuming caseloads when needed
- Conducting assessments and intakes
- Leading team meetings and providing training
- Ensuring compliance and quality of care
- Coordinating care with external providers
- Supervising staff and providing orientation and ongoing education
- Participating in staff development and quality programs
Ideal Candidate Qualifications:
- Current RN license in good standing
- Graduate of an accredited nursing program
- Experience in home health, pediatrics, or office healthcare preferred

Dallas Branch Specifics:
- Office-based role
- Coordinate office support, nurse walk-ins, IPEA, CVL, weekend IPEA, and intakes
- Lead re-education of field staff
- Oversee ODOO and branch supplies
- Support new CCM training and shadowing
- Maintain dashboards for CCR, follow-up, supervisory visits, and annual evaluations
