Position Summary:
We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our team. The LCSW will work closely with our primary care providers (PCPs) to deliver integrated behavioral health and counseling services directly in the clinic. The LCSW will play a vital role in providing behavioral health, addressing social determinants of health, and supporting patients and families in achieving their health goals. The ideal candidate thrives in a collaborative setting, has strong clinical skills, and is passionate about addressing the behavioral and social needs of patients.
Key Responsibilities:
Behavioral Health & Counseling Services:
Provide brief, evidence-based counseling and therapeutic interventions for patients experiencing depression, anxiety, stress, trauma, or chronic illness-related concerns.
Offer ongoing, short-term counseling services to support patient needs and goals.
Utilize motivational interviewing, CBT, ACT, and other therapeutic modalities as appropriate for primary care settings.
Work on-site with PCPs to ensure warm handoffs and seamless integration of behavioral health services into patient care.
Provide real-time consultation to PCPs during patient visits, offering recommendations for behavioral health and psychosocial needs.
Participate in team huddles and care planning meetings to ensure coordinated and patient-centered care.
Social Determinants of Health (SDOH):
Screen patients for SDOH challenges, including housing, transportation, food security, and healthcare access.
Connect patients with community resources and provide advocacy and follow-up to address barriers to care.
Collaborate with community partners to improve access to supportive services for patients.
Crisis Intervention & Support:
Provide on-site crisis intervention for patients experiencing acute mental health or psychosocial crises.
Facilitate appropriate referrals for higher levels of care, including substance use treatment or psychiatric services, as needed.
Documentation & Education:
Maintain accurate and timely documentation in the electronic health record (EHR).
Educate patients, families, and caregivers on coping strategies, stress management, and available resources.
Support staff training on SDOH screening, documentation, and referral processes.
Qualifications:
Active Oregon LCSW license in good standing.
Minimum of 2 years of clinical experience in a healthcare or primary care setting preferred.
Familiarity with integrated care models, warm handoffs, and evidence-based therapeutic techniques.
Strong knowledge of local community resources and social service systems in Oregon or willingness to learn and engage with community resources.
Excellent communication, interpersonal, and team-based collaboration skills.
Proficiency in using EHR systems for documentation.
Preferred Qualifications:
Experience delivering brief, solution-focused therapy in primary care or medical settings.
Familiarity with addressing health equity and working with diverse patient populations.
Bilingual in English and Spanish (or other languages) is a plus.
