Job summary

Polaris Rehabilitation & Care Center in Cheyenne, WY seeks an experienced Charge Nurse (RN/LPN) to lead nursing operations on a shift, supervise staff, and ensure resident care and regulatory compliance.

You will provide direct care, coordinate plan of care, administer medications, mentor CNAs, document in EMR, and communicate with physicians and families. Competitive benefits and a sign-on bonus up to $10,000 are included.

Qualifications

• Graduate of an accredited nursing program.
• Current, active RN or LPN license in the State of Wyoming.
• Current CPR or Basic Life Support (BLS) certification.

Responsibilities

• Lead and coordinate nursing operations during the assigned shift.
• Provide direct resident care per care plans and physician orders.
• Supervise, delegate, and support CNAs and nursing staff during the shift.
• Monitor resident conditions and ensure timely clinical interventions.
• Administer medications and treatments within scope of practice.
• Maintain accurate EMR documentation and communicate changes promptly.
