Polaris Rehabilitation & Care Center in Cheyenne, WY is seeking an experienced Charge Nurse (RN or LPN) to lead nursing operations on a shift. You will supervise staff, coordinate resident care, and ensure compliance with CMS and state regulations while delivering compassionate, high‑quality care.

Qualifications

• Graduation from an accredited nursing program.
• Current, active RN or LPN license in good standing in Wyoming.
• CPR/BLS certification.
• Experience in skilled nursing or post-acute care preferred.
• Charge Nurse or supervisory experience preferred.
• Knowledge of CMS regulations, infection prevention, and long-term care standards.
• Strong leadership and communication skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Commitment to resident-centered care and staff development.

Responsibilities

• Lead and coordinate nursing operations during the assigned shift.
• Provide direct nursing care per care plans and orders.
• Monitor resident conditions and intervene as needed.
• Administer medications and treatments within scope.
• Supervise CNAs and nursing staff during shift.
• Provide coaching and problem solving for staff.
• Ensure accurate EMR documentation.
• Communicate changes to physicians, families, and team.
• Assist with admissions, transfers, and discharges.
• Promote CMS/Wyoming regulations and infection prevention.
• Participate in QAPI and safety programs.
• Respond to emergencies with sound clinical judgment.
• Foster a positive, resident-centered work environment.
