Job summary

Polaris Rehabilitation & Care Center in Cheyenne, WY is seeking a compassionate Licensed Practical Nurse (LPN) to join our clinical team. You will provide direct nursing care, administer medications, monitor residents, and collaborate with RNs, physicians, and other staff to deliver safe, high-quality, resident-centered care.

This DAY shift role offers a rotational schedule and a $10,000 sign-on or relocation bonus.

Qualifications

• Graduate of an approved practical nursing program.
• Current Wyoming LPN license in good standing.
• Current CPR or BLS certification.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Strong clinical observation, critical thinking, and communication skills.

Responsibilities

• Provide direct nursing care per care plans and physician orders.
• Monitor resident conditions and report changes to the RN or medical team.
• Administer medications and treatments within scope of practice.
• Support resident assessments and daily clinical operations.
• Supervise CNAs and support staff.
• Maintain accurate documentation in the EMR.
• Follow infection control and safety protocols.
• Respond to urgent resident needs and changes in condition.
• Promote resident dignity, comfort, and quality of life.
• Ensure compliance with regulations and facility policies.
• Participate in quality improvement and care planning.
