Job summary

Volunteers of America Colorado is looking for a Case Manager (Team Lead) to direct and facilitate delivery of support services. The role includes needs-based assessments, outreach, case management, and coordinating services with a trained team.

The Case Manager 4 (Team Lead) will maintain a caseload and mentor junior staff to promote best practices. Responsibilities include mentorship, staff training, and ensuring documentation accuracy while collaborating with supervisors to advance program

Qualifications

• Bachelor’s degree in human services, social work, or a closely related field.
• 1 year of direct, full-time case management experience.
• Colorado driver’s license and state-mandated automobile insurance.
• Personal vehicle for work-related travel (mileage reimbursement).
• Complete agency and program credentialing within 12 weeks of hire.

Responsibilities

• Provides case management services to program clients.
• Identify and serve the needs of participant households within VOA programs.
• Understand and implement program requirements and funder standards.
• Conduct community outreach to identify eligible households facing housing barriers.
• Mentor and support team members; lead training and staff development.
• Maintain confidential, accurate client documentation and records.
