Qualifications

• Requires a Bachelor’s degree in psychology, social work, mental health, or a related field.
• Must be employed by Agape Network as a Targeted Case Manager for at least 3 months with a satisfactory evaluation.

Responsibilities

• Receive timesheets and progress notes from case managers weekly.
• Monitor completion of TCM assessments and paperwork such as service plans and case notes.
• Submit weekly timesheets according to billing requirements.
• Prepare and submit Employee Performance Reports weekly to the supervisor.
• Provide training, consultation, and problem-solving support to Case Managers.
• Consult on difficult cases and offer second opinions and guidance.
• Perform other related duties as assigned by the Case Manager Supervisor.
