Job summary

Theagapenetwork is seeking a Case Manager to coordinate client services for adult clients according to the treatment plan. You will manage intake assignments, schedule court appearances, doctor appointments, family therapy sessions, and discharge referrals while maintaining confidentiality.

Responsibilities include maintaining active caseloads, acting as liaison between courts, families, and outside agencies, documenting reports, and participating in treatment team meetings to support program

Qualifications

  • Bachelor’s degree in psychology, social work, mental health or related field.
  • One year’s experience working in the mental health field serving diverse population.
  • Possess case manager certification or obtain certification within three months of employment.
  • Maintain an active driver license.

Responsibilities

  • Maintain a caseload of active clients who require case management in such areas as programming court appearances, doctor appointments, family therapy sessions, and discharge referrals.
  • Act as liaison between courts, families, and outside agencies.
  • Provide case management services in accordance with the service plan as appropriate to the program.
  • Coordinate and meet with the members of the Treatment Team and provide them the appropriate reports prior to meeting.
  • Completes documentation and reports as necessary.
  • Provides on-call services as needed.
  • Attends staff meetings as necessary and participates as a team member.
  • Upholds total confidentiality with respect to clients in all instances.
  • Ensure that each client is registered for food stamps. Follow up on the completion of the process, working jointly with administrative Case Manager.
  • Maintain open communication with family, courts, and others as necessary.
  • Ensure that authorization for Medicaid clients is in on time.
  • Monitor the billing data/units for Medicaid.
  • Maintain client case management logbook in accordance with Program/Contract guidelines.
  • Attends training and in-services as required.
  • Performs other duties as appropriately and assigned.
