The Lead Case Manager will maintain a caseload and be responsible to receive case manager’s documentation on a weekly basis in order to approve and process services for billing and payment to rendering provider. Each Targeted Case Manager will be assigned specific clients at time of intake and be required to program and monitor such things as basic needs, academic issues, court appearances, doctor appointments, clinical sessions and discharge referrals. The Targeted Case Manager will also maintain open communication with family, courts and others as necessary. The Targeted Case Manager will meet with members of the Treatment Team as needed.
Bachelor’s degree in Psychology, social work, mental health or related field. Be employed by Agape Network as a Targeted Case manager for at least 3 months and have a satisfactory evaluation during this period.
