Qualifications

• Two years of supervised work experience in providing individual/family therapy is preferred.
• Bilingual is preferred.
• Must have reliable transportation.
• Familiarity with Miami-Dade County.

Responsibilities

• Administer evidence-based assessments as needed.
• Conduct Needs Assessment.
• Develop Service Plans in collaboration with the consumer and family.
• Conduct monthly Service Plan reviews.
• Document all clinical services and activities in accord with licensing, Medicaid, and Joint Commission standards and agency policy and procedure.
• Conduct individual case management sessions.
• Provide vocational education and skills building to the consumer as needed.
• Coordinate consumer and family care with the treatment team and external providers.
• Develop discharge plans in coordination with the treatment team.
