Job summary

Heritage Health Network leads an ECM program as a Lead Care Manager in a hybrid role, combining remote collaboration with in-field member support.

You will coordinate care, develop personalized plans, and work with healthcare teams to improve outcomes while documenting progress and advocating for members.

Qualifications

• Experience in care or case management; community-based preferred.
• Knowledge of Medi-Cal and ECM strongly preferred.
• Strong communication and interpersonal skills.
• Ability to work in a team-oriented environment.
• Willingness to travel within Orange County.
• Bilingual (English/Spanish) preferred.

Lead Care Manager (ECM) -- Hybrid Role

Join Heritage Health Network and make a meaningful impact in your community as a Lead Care Manager supporting members through our Enhanced Care Management (ECM) program.

This is a hybrid position (2 days remote, 3 days in the field) , giving you the flexibility to work from home while building strong, in-person connections with members across Low Desert regions.

Key Responsibilities

• Serve as the primary point of contact for members and coordinate their care
