Job summary

Heritage Health Network (HHN) is seeking a Lead Care Manager to support members through Enhanced Care Management (ECM) in a hybrid role. The position combines field visits (2 days) and remote work (3 days) to coordinate personalized care plans across Low Desert regions near Palm Desert.

Responsibilities include serving as the primary contact, collaborating with healthcare teams, and driving education, advocacy, and engagement while travel within Orange County is expected.

Qualifications

• Experience in care or case management (community-based preferred).
• Knowledge of Medi-Cal and ECM strongly preferred.
• Strong communication and interpersonal skills.
• Ability to work in a team-oriented environment.
• Willingness to travel within Orange County.
