Job summary

Heritage Health Network seeks a Lead Care Manager to support members in our Enhanced Care Management program. This hybrid role blends 2 days remote and 3 days in the field, serving High Desert regions while building strong, in-person connections with members.

Responsibilities include coordinating care, creating personalized plans, collaborating with healthcare teams, and addressing gaps in care. Travel within Orange County is expected; bilingual English/Spanish is preferred.

Qualifications

• Experience in care or case management, community-based preferred.
• Knowledge of Medi-Cal and ECM strongly preferred.
• Excellent communication and interpersonal skills.
• Able to work in a team-oriented environment.
