Heritage Health Network in Victorville, CA is seeking a Lead Care Manager to guide members through our Enhanced Care Management program in a hybrid role that blends remote coordination with in-person field work.

You will develop personalized care plans, coordinate with providers, educate and advocate for members, and ensure timely documentation while traveling within the Orange County region as needed.

Competenze

• Experience in care or case management (community-based preferred).
• Knowledge of Medi-Cal and ECM strongly preferred.
• Strong communication and interpersonal skills.
• Ability to work in a team-oriented environment.
• Willingness to travel within Orange County.
• Bilingual English/Spanish preferred.

This is a hybrid position (2 days remote, 3 days in the field) , giving you the flexibility to work from home while building strong, in-person connections with members across High Desert regions.
